A step-by-step decision

A doctor change and an insurance change are different decisions.

You may be able to select a different primary-care clinician within your existing coverage. Your plan controls network and PCP-selection rules; the office verifies participation and explains its scheduling process.

Confirm coverage and effective dates

Call the number on your plan card to ask whether PCP selection is required and when a change becomes effective. Then call the office with the exact plan information.

Protect medication continuity

Bring a current medication and pharmacy list. Do not stop or change medicines on your own. Ask your existing clinician about refills needed before the transition.

Keep specialists connected

Changing a PCP does not automatically change every specialist. Bring specialist names, contact details, referral status, and authorization documents. The plan determines network and authorization rules.

Transfer useful records

Ask the new office which records are most useful and how authorization should be completed. Bring recent summaries, labs, imaging reports, and vaccine information if available.

Understand costs

Copays, deductibles, and covered services come from your plan. The office can verify participation but cannot promise what your plan will pay.
