A step-by-step decision

Ask the office what authorization is required to discuss care with a family member. Being listed as an emergency contact does not necessarily authorize access to health information.
Write down the patient’s goals, changes in health, refill needs, mobility or language needs, and the questions that must be answered at the first visit.
Online tools are optional. Patients and authorized caregivers can call the office for help with the clinic’s process. Staff will explain what can be handled by phone.
